What do you like?
The program is really easy to use and very intuitive. The clinical summary screen is great. The support staff seem really helpful. The e-prescribing function is quick and easy to use. It's great being able to create custom treatment plans with our own wording when desired.
What needs work?
The patient portal needs some work. The medical history via the portal is incomplete. As a result we are still having patients fill out their history and review of systems on paper and then we manually type it in. Huge waste of technician time. They really need a full ROS that meets the qualifications for a comprehensive exam on the portal.

The exam screen does not load all the previous exam findings. For example, if I note a retinal detachment repair one month and then load my previous findings at the next exam, this finding does not show up on the exam entry screen. Same is true for all slit lamp findings. CD does carry over though.

What do you like?
After about 6-8 months with our first EHR/PM package, I asked the staff to list 3 reasons we should keep it. They could only list one-it generated outgoing 837 files that our clearing house site could import. But the software could not import the 835 electronic remittance files. We were expected to log all of that in by hand (we never did). We acquired this package without a concurrent PM system-did not want 2 simultaneous learning curves. We did not purchase conversion services for the relatively few (6-8 months worth) records from the older system. Modernizing Medicine's EMA was a replacement, both were (are) cloud based, but the other system was comparatively very slow. 'EMA' is zippy fast. It codes for 99xxx as well as 92xxx procedure codes as well-the old one only coded for 99xxx. I like that the contact lens parameters are already loaded in the system such choices as BC, Cyl pwr, brand and type are all there. (The old system required us to load in all of the CL types and parameters!!!!!!!!!!) It uses a unique 'Virtual exam room' which does take getting used to but it gets the job done well and quickly. Start up is $3k and $300/mo. Support is attentive and fast via email and via support site. Very extensive diagnosis code list autofill as you type a name or parts of an ICD code. There are of course a few nuances that I would like addressed but all in good time. An absolute requirement was that our package be able to run natively on a Mac based office, and it runs on the iPad as well. And did I say it is 'FAST'? This package originated from EMA-Dermatology and has expanded rapidly to include Optometry/Ophthalmology and other specialties. It has built in handout (customizable) sheets for many common conditions. Good eRx engine also. EMA adds coding modifiers also. Haven't used the scheduler, since we are used to Google calendar. Also, very important, Modernizing Medicine IS working to implement ICD-10-CM coding whereas I have heard that less than half of the EHR packages available intend on doing so. Meaningful Use attestation for 2013 was accomplished even after implementing EMA in late August 2013.
What needs work?
There are little bits and pieces that need work. For example, you can fax a consult note out of the yet to be finalized record, and it shows in an outgoing fax processing list. But often faxes are unsuccessful due to 'no answer' or 'line busy' type issues, but there is not a 're-send' fax button beside the failed fax item, if the file has already been finalized. That would be a good feature to add. One has to be vigilant with additional procedure codes and diagnoses selected vs to what is shown on the ready to finalize visit note. For example I have selected an intermediate field (92082) EMA shows a 92083 in the visit note page. Little things. Will generate a support ticket when I am bothered enough by it-but not so bothered right now. ModMed plans on adding equipment interfacing but we have not found that to be an issue. Pre test info is added once and can be populated elsewhere as desired. Other outputs such as fundus photos and fields are imported into the document management section easily enough.
