EW template has duplicate info

The template that I use has been refined over the 3+ years that I have used EW and it works for my office though I am not completely pleased with its efficiency. The information that I have included in the template is to record patient data and to satisfy the insurance industry for "Patient History" and ROS "Review of Systems". I find that mostly the ROS is just a restatement (arduous task too) of the patient history. If patient is on Lisinopril, then medical Hx is HTN and also in ROS it is HTN. Is this duplication of info necessary, is my template poorly designed?

Comments and suggestions please.

I have refined my templates to reduce duplicate entry. If you look at some other doctor's paper exams there is far less data than an examwriter template exam.

Anyway, what I do is put in the review of systems this statement, "Systems reviewed. For positive finding see Patient History above." I think that meets the requirements without leaving out any data or having to enter it twice.

Shaune, that seems like a great solution, but I wonder if it will pass the "VSP" audit test. What do you think Adam?

you know I'm not really sure. OM has lots of "blanket" statements like that, so who knows really. Anyone ever been audited using OM on this board?

I was audited, had to print and send 10 records to VSP and passed, but did the conventional exam with both ROS and medical history, a duplication to be sure and a pain.
